Biometric Screenings

The 2023-2024 Wellness Program has been simplified!

Incentives are earned based on participation and their wellness actions completed before the October 31 deadline. In past wellness years, our program was mostly out-comes based and employees tried to meet certain health measures to earn incentives. We have simplified our program and made it easier for employees to earn incentives offered. 

Wellness Actions - Biometric Screening and Cotinine (Tobacco) Screening

To earn wellness incentives offered, employees must complete a biometric screening and test negative for Cotinine (tobacco) within the wellness year. These actions and results need to be submitted to our Employee Wellness Center by October 31, 2024.  

If you are a Uniformed Fire & Police employee: Your biometric screening and Cotinine data is captured during your annual physical with the City's Provider. If you are on modified duty or you have work-restrictions that prevent you from completing your annual physical in time with the wellness program, schedule a regular personal health biometric screening and cotinine appointment with the Wellness Center to get this done. Options For Completing a Biometric Screening and Cotinine Testing:

  • Visit the Employee Wellness Center: Employees can schedule an appointment to complete biometrics and Cotinine testing.
    • Appointments can be made by setting up an account at MyPremisehealth.com
    • When selecting your appointment type, select "Biometric Screening Visit" when scheduling through the mobile app or website.
    • There are notes you can input into the appointment. If you want to be tested for Tobacco (Cotinine), please indicate this in the notes or let the Wellness Center staff know at your appointment.

  • Visit your Primary Care Physician and complete the Physician Screening Form: The physician form can be found here: 2024 City of Frisco Physician Form - Premise Health (PDF)
    • Take this form to your doctor to complete during your annual physical or routine bloodwork. Instructions on how to submit the form to the Employee Wellness Center are located on the form as well as here: Adding a Document or Provider Form in Epic (PDF)
    • Cotinine Testing: If you use your doctor to complete your biometric screening, then you have one additional step to get your Tobacco status submitted. You will need to schedule an appointment with the Employee Wellness Center to complete Cotinine testing (see first option). Cotinine testing is quick and should not take more than 10-15 minutes.

The Employee Wellness Center manages our Wellness Program. They send the City incentive reports to let us know the incentives earned by employees based off their wellness actions submitted. The reports only indicate the incentives you earned, it does not share any other information regarding your health.

The Wellness Center utilizes the Epic medical charting system where they store and secure records. The City has no access to this as it is private. You can view your Epic records through your www.mypremisehealth.com portal.
